Office Locations
Scottsdale, Arizona
Our Scottsdale office is located on the HonorHealth Thompson Peak medical campus. The campus is just off Scottsdale Road, behind the Discount Tire corporate office on Thompson Peak Parkway.
The office is in the Grayhawk Medical Plaza building, just north of the hospital. We are located on the second floor, Suite 210. Turn left when you exit the elevators- we will be the last office on your left
HonorHealth Scottsdale Thompson Peak Campus
There are two SMIL (Scottsdale Medical Imaging) sites on campus - one in the Grayhawk building, one in the Thompson Peak building.
Yavapai County, Arizona
Our office in Yavapai County is located at 3198 Windsong Dr. in Prescott Valley. This office is located in the Windsong Medical Plaza - at the intersection of Windsong and Florentine Dr. As part of our collaboration with HonorHealth, the name on the signage indicates the HONORHEALTH Medical Group.
The office is open Monday through Friday, from 8AM to 5PM. Our PA Wes Wagar is in the office on a full-time basis to meet the needs of our orthopaedic patients in Prescott, Prescott Valley and the surrounding communities.

Payments
All co-payments and deductibles are due at the time of service. This includes payment for any surgical procedures, which is usually taken care of in advance. We accept cash, check, and Visa and MasterCard credit cards.
Cancellations
In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.
